Abstract

Purpose

Organizations working in high-hazard environments contribute significantly to modern society and the economy, not only for the valuable resources they hold but also for the indispensable products and services they provide, such as power generation, transportation and defense weapons. Therefore, the main purpose of this study is to develop a framework that outlines future research on systems safety and provides a better understanding of how organizations can effectively manage hazard events.

Design/methodology/approach

In this research, we developed the high hazard theory (HHT) and a theoretical framework based on the grounded theory method (GTM) and the integration of three established theoretical perspectives: normal accident theory (NAT), high reliability theory (HRT) and resilience engineering (RE) theory.

Findings

We focused on the temporal aspect of accidents to create a timeline showing the progression of hazard events and the factors contributing to safety and hazards in organizations. Given the limitations of the previous theories in providing a coherent explanation of hazard event escalation in high-hazard organizations (HHOs), we argue that the highlighted theories can be more complementary than contradictory regarding their standpoints on disasters and accident prevention.

Practical implications

A proper appreciation of the hazard nature of organizations can help reduce their susceptibility to failure, prevent outages and breakdowns of systems, identify areas for improvement and develop strategies to enhance performance.

Originality/value

By developing HHT, we contribute to systems safety research by developing a new, refined theory and enrich the theoretical debate. We also expand the understanding of scholars and practitioners about the characteristics of organizations working in high-hazard environments.

Abstract

Purpose

High-reliability performance and high-hazard are intertwined in High-Reliability Organizations (HROs) operations; these organizations are highly safe, highly hazardous and highly significant for the modern society, not only for the valuable resources they have, but also the indispensable services they provide. This research intend to understand how HROs could produce high quality performance despite their challenging and demanding contexts. The research followed an emic approach to develop an organizational framework that reflects the contribution of the seeming traits of the organizations to the operations safety based on the workers point of views about the safety of workstations.

Design/methodology/approach

This research adopted mixed methods of in-depth interviews and literature review to identify the structural characteristics of high-reliability organizations (HROs) embedded in the organizations studies and developed a theoretical based structural framework for HROs. Furthermore, a systemic literature review was adopted to find the evidence from the organizations literature for the identified characteristics from the interviews from the first stage. The setting for this study is six Chinese power stations, four stations in Hubei province central China and two stations in the southern China Guangdong province.

Findings

The organizational framework is a key determinant to achieve high-reliability performance; however, solely it cannot explain how HROs manage the risks of hazard events and operate safely in high-hazard environments. High-reliability performance is attributed to the interaction between two sets of determinants of safety and hazard. The findings of this research indicate that HROs systems would be described as reliable or hazardous depending on the tightly coupled setting, complexity, bureaucracy involvement and dynamicity within the systems from one hand, and safety orientation, failure intolerance, systemwide processing, the institutional setting and the employment of redundant systems on other hand.

Originality/value

The authors developed an organizational framework of organizing the safety work in HROs. The applied method of interviewing and literature review was not adopted in any other researches.

Abstract

Purpose

Maintaining a safe and available supply of blood requires a mindfully coordinated supply chain (SC) and is fundamental to the effective operation of health systems across the world. This study investigates how blood supply chain (BSC) actors demonstrate collective mindfulness (CM) principles in their operations and how these demonstrations lead to improvements in blood safety and availability (BSA) in different operational contexts.

Design/methodology/approach

Six case studies drawn from two contrasting BSCs, the UK and Indonesia, which differ in structure and regulation are investigated in this research. Qualitative data are collected and analysed using template analysis.

Findings

The cases reveal how the CM principles are demonstrated in the supply chain context in a range of operational conditions and their impact on BSA. The BSC actors in the more centralised and tightly regulated cases display more behaviours consistent with more of the CM principles over a greater range of operational conditions compared to those in the more decentralised and loosely regulated cases. As such, more improvements in BSA are found in the former compared to the latter cases.

Originality/value

This paper is considered the first to investigate the demonstration of CM principles at the SC as opposed to the single organisational level. It proposes an alternative approach to understanding and evaluating reliability performance using behavioural rather than statistical principles.

Abstract

Purpose

Recent lapses in the management of high hazard organizations, such as the Fukushima event or the Deepwater Horizon blast, add considerable urgency to better understand the complicated and complex phenomena of leading and managing high reliability organizations (HRO). The purpose of this paper is to offer both theoretical and practical insight to further strengthen reliability in high hazard organizations.

Design/methodology/approach

Phenomenological study based on over three years of research and thousands of hours of study in HROs conducted through a scholar-practitioner partnership.

Findings

The findings indicate that the identification and the management of competing tensions arising from misalignment within and between public policy, organizational strategy, communication, decision-making, organizational learning, and leadership is the critical factor in explaining improved reliability and safety of HROs.

Research limitations/implications

Stops short of full-blown grounded theory. Steps were made to ensure validity; however, generalizability may be limited due to sample.

Practical implications

Provides insight into reliably operating organizations that are crucial to society where errors would cause significant damage or loss.

Originality/value

Extends high reliability research by investigating more fully the competing tensions present in these complex, societally crucial organizations.

Abstract

Purpose

In light of and due to the spike in concern regarding high hazard industries, in general, and nuclear power plants (NPPs) in particular, resulting from the Japanese earthquake and crisis at Fukushima, the purpose of this paper is to offer an innovative organizational development (OD) intervention that may enhance safety and operational performance directed at these critical organizations.

Design/methodology/approach

Drawing on and integrating key elements of strategy, leadership coaching and development and assessment, the authors describe and detail an intervention designed to bring a troubled NPP to a state of reliability.

Findings

It was found that performance improved in a relatively short amount of time from implementing this OD tool.

Practical implications

The findings contained herein may apply to any organization aiming to improve on safety and operational performance.

Originality/value

The paper's findings should appeal to high hazard and high reliability organizations, such as those found within the energy industry, that must continuously strive toward improved operational and safety performance.

Abstract

Purpose

Extreme high temperatures are a significant feature of global climate change and have become more frequent and intense in recent years. These pose a significant threat to both human health and economic activity, and thus are receiving increasing research attention. Understanding the hazards posed by extreme high temperatures are important for selecting intervention measures targeted at reducing socioeconomic and environmental damage.

Design/methodology/approach

In this study, detrended fluctuation analysis is used to identify extreme high-temperature events, based on homogenized daily minimum and maximum temperatures from nine meteorological stations in a major grassland region, Hulunbuir, China, over the past 56 years.

Findings

Compared with the commonly used functions, Weibull distribution has been selected to simulate extreme high-temperature scenarios. It has been found that there was an increasing trend of extreme high temperature, and in addition, the probability of its indices increased significantly, with regional differences. The extreme high temperatures in four return periods exhibited an extreme low hazard in the central region of Hulunbuir, and increased from the center to the periphery. With the increased length of the return period, the area of high hazard and extreme high hazard increased. Topography and anomalous atmospheric circulation patterns may be the main factors influencing the occurrence of extreme high temperatures.

Originality/value

These results may contribute to a better insight in the hazard of extreme high temperatures, and facilitate the development of appropriate adaptation and mitigation strategies to cope with the adverse effects.

Abstract

Purpose

The purpose of this paper is to enhance understanding low pay dynamics of Australian employees, with a focus on the determination of low pay duration.

Design/methodology/approach

The study draws on a representative longitudinal survey of Australian households to provide empirical findings from both descriptive analysis and econometric modelling.

Findings

The results show that workers who have entered low pay from higher pay also have a higher hazard rate of transitioning to higher pay; and those who have entered low pay from non-employment are more likely to return to non-employment. Union members, public sector jobs and working in medium to large size firms tend to increase the hazard rate of transitioning to higher pay, while immigrants from non-English speaking countries and workers with health problems have a lower hazard rate of moving into higher pay. There is some evidence that the longer a worker is on low pay, the less likely he or she is to transition to higher pay.

Originality/value

This study addresses an information gap regarding the determination of low pay duration. The findings help identify workers who are at high risk of staying on low pay or transitioning into non-employment and are therefore informative for developing targeted policy to help the low paid maintain employment and/or move up the earnings ladder. The results also suggest that policy intervention should take place at an early stage of a low pay spell.

Abstract

By utilizing a large sample of firms during the period 2006–2017, the author determine which types of firms are more likely to go bankrupt. The author shows that over-leveraged firms have significantly higher probability of going bankrupt, which highlight the importance of the concept of optimal corporate capital structure. The author finds that private firms and export-oriented firms experience lower hazard rates. Proposed hazard statistical model highlights that more profitable firms, firms with better liquidity, firms with more tangible assets and larger firms all have statistically higher survival rates. The author finds that bankruptcy rates are the lowest among service firms and the highest in construction industry. Ownership variables indicate that state-owned firms, firms with foreign ownership and firms, owned by holdings, are less likely to fail, all else equal. Finally, the author demonstrates that proposed statistical model successfully predicts the probability of bankruptcy. The mean cumulative hazard function for a group of surviving firms is statistically significantly lower compared to a group of failing firms. In order to survive in a long run, firm’s management should especially be aware of their optimal capital structure and use rather less leverage than going over the sustainable level.

The purpose of this paper is twofold. First, it assesses motives for intended mobility among academics in institutions of higher education. Second, it investigates gender differences. Women have twice the intention to leave their institution than men during their first few years, but this difference narrows with seniority. Women report monetary reasons such as salary and promotion opportunities, as well as non-monetary reasons such as spousal employment to motivate their intended mobility. Gender differences across the reasons are minor once one controls for tenure status.

Purpose

Many researchers have investigated the determinants of workers’ risk-taking/unsafe behaviours as a way to improve safety management and reduce accidents but there has been a general lack of research about workers’ risk information seeking behaviours or their source preferences for risk information. The purpose of this paper is to investigate whether occupational risk information source preference was risk independent (i.e. whether workers prefer to receive occupational risk information from proximal sources like supervisors and workmates regardless of the nature of the risk or the source's expertise regarding that risk, or if they discriminated between information sources based on the type of risk being considered).

Design/methodology/approach

Data were collected from 106 frontline construction workers who were recruited from a single building site within the UK with the help of the safety officer on site. The source from which workers preferred to receive information about a range of risks was measured using a ranking exercise. Specifically, workers were asked to rank five occupational sources (HSE, safety manager, project manager, supervisor, workmates) according to how much they preferred each one to deliver information about eight different risks (asbestos, back pain, site transport, heights, slips/trips, housekeeping, and site-specific and job-specific risks).

Findings

The paper found that supervisors and safety managers were the most preferred sources of risk information overall, but a correspondence analysis suggested that workers’ risk information source preference is risk dependent and might be driven by source expertise.

Practical implications

The findings have important practical implications for the role of safety managers in risk communication and for building trust within high-hazard organisations.

Originality/value

To the author's knowledge, this is the first study to investigate risk information source preferences in an occupational setting.
