Abstract

Purpose

This study aims to explore how leadership, management practices and organisational cultures have changed in low and high-performing mental health (MH) providers between 2015 and 2020 in the English National Health Service.

Design/methodology/approach

The authors used a qualitative case study design comprising a purposeful sample of two low-performing and two high-performing MH providers, based on semi-structured interviews with 60 key informants (mostly internal to the organisation with some external informants from local Clinical Commissioning Groups).

Findings

The authors found major differences regarding leadership, management and organisational culture between low and high performing MH providers in 2015/2016, and that the differences had diminished considerably by 2019/20. In 2015/16, low performing providers were characterised by a “top-down” style of leadership, centralised decision-making and “blame cultures”. In contrast, the high performing providers were characterised as having more distributed, collaborative and inclusive styles of leadership/management, with open and supportive cultures. As the low performing providers changed and adapted their styles of leadership and management and organisational culture over the five-year period, they more closely resembled those of the high performing trusts.

Originality/value

To the best of the authors’ knowledge, this is the first study to explore the relationship between changing organisational factors and the performance of MH care providers. It provides evidence that it is possible for radical changes in leadership, management and organisational culture to be enacted over a relatively short period of time and that such changes may help low performing providers to turnaround their underperformance.

Abstract

Purpose

This study aims to compare and contrast the core organisational processes across high and low performing mental health providers in the English National Health Service (NHS).

Design/methodology/approach

A multiple case study qualitative design incorporating a full sample of low and high performing mental health providers.

Findings

This study suggests that the organisational approaches used to govern and manage mental health providers are associated with their performance, and the study’s findings give clues as to what areas might need attention. They include, but are not limited to: developing appropriate governance frameworks and organisational cultures, ensuring that staff across the organisation feel “psychologically safe” and able to speak up when they see things that are going wrong; a focus on enhancing quality of services rather than prioritising cost-reduction; investing in new technology and digital applications; and nurturing positive inter-organisational relationships across the local health economy.

Originality/value

Highlights considerable divergence in organisation and management practices that are associated with the performance of mental health trusts in the English NHS

Abstract

The South African government’s objectives relating to tobacco, primarily deterrence and revenue generation, have since the early 1970s not been maintained. The real value of tobacco taxes fell over the 20‐year period 1970 to 1990 and, as a result, consumption steadily increased. Since 1990, however, the government has, despite vociferous opposition from the industry, embarked on a stronger programme of tobacco control and has increased excise taxes in real terms. Uses a simulation model of the government’s non‐optimal tax policy to show that revenue could have increased by 2 per cent, had the government maintained the real value of excise duties. These findings suggest that, despite government moves in the right direction, there is still much room for a stronger tobacco taxation policy in South Africa that will enhance government revenue and contain consumption.

Abstract

Purpose

The purpose of this paper is to explore the link between one aspect of primary care in England – the annual review by general practitioners for dementia patients – and length of hospital stay (LoS). The annual review should identify the needs of both patients and carers and co-ordinate services across health and social care to address those needs. If this is done well, timely discharge from hospital may be facilitated.

Design/methodology/approach

The study uses linked national data from 2006/2007 to 2010/2011 on over 36,000 patients, employing sophisticated statistical techniques to isolate the effect of the annual dementia review on LoS.

Findings

Hospital patients discharged to the community have significantly shorter stays if they are cared for by practices that reviewed a higher percentage of their patients with dementia. However, this effect is small and is not evident for patients discharged to care homes or who died in hospital. Longer LoS is associated with a range of co-morbidities, markers of low availability of social care and with intensive provision of informal care.

Practical implications

Although the dementia review has only a modest effect on LoS, the components of the review could improve the health and well-being of those with dementia and their carers.

Originality/value

The study is the first to employ a robust methodology to investigate the impact of the annual dementia review on hospital LoS, an important aspect of the interface between primary and secondary care. There are implications for clinical and financial aspects of health and social care policy.

Abstract

This paper investigates the influences of temporary contracts along several dimensions of well-being (physical and mental health, self-assessed health and happiness) for young Italian workers. Our paper contributes to the literature exploring some new aspects of the relationship between temporary jobs and well-being in a country not frequently analysed in previous literature. We focus on the gender gap in the well-being consequences of non-permanent jobs, the influence of financial support by family in reducing well-being effects caused by temporary contracts and the interaction between gender gap and family support. We find that temporary contracts are damaging in terms of psychological health and happiness mostly for young men and individuals without family economic support. On the other hand, women’s mental health is not affected by temporary contracts and they are even better off in terms of their mental health and well-being when receiving family economic support.

Abstract

This paper examines some of the key features of social health insurance systems by drawing on experiences in Germany, Switzerland, France and The Netherlands. These countries have all implemented a variety of reforms, including some competition between health insurers in order to meet some of their health care objectives. The paper highlights some of the strengths and weaknesses inherent in these systems and how they perform on a number of criteria and suggests a number of trade‐offs which policymakers will have to grapple with to attain some of their (often competing) health system goals of efficiency, choice, solidarity and equity. This paper should provide useful information for countries with health care systems in transition or those considering adopting aspects of social health insurance systems.

Abstract

Purpose

This paper aims to analyze how performance may be defined in the context of a religious organization. The authors do so by studying the case of a Benedictine abbey.

Design/methodology/approach

Using an etymologically derived understanding of performance, and predominantly based on the Regula Benedicti (the central guideline for Benedictine monks), the authors first conceptually develop an understanding of performance in Benedictine abbeys. The authors then apply this understanding in a single case study.

Findings

The authors found that in order to comply with the Benedictine mission laid out in the Regula Benedicti, Benedictine abbeys need to balance sacred and secular goals. The authors also derived six key actions in order to accomplish these goals. The case study shows that an imbalance in these key actions may cause severe (financial) distress.

Research limitations/implications

The study provides an alternative framing of the term “performance” and further evidence that only a combined pursuit of sacred and secular goals seems useful for religious organizations. Researchers interested in religious organizations might find the conceptual approach and findings useful to analyze performance in such organizations.

Practical implications

Benedictine abbeys and other religious organizations may find the analysis valuable to critically analyze their current strategies and focal activities. Moreover, this paper's results might also be worthwhile for other faith-based or third-sector organizations when seeking an alternative framing of performance.

Originality/value

This paper provides a new framing of “performance” and is the first to analyze what performance might mean in the context of a Benedictine abbey.
