Abstract

Purpose

A key question in the provision of public health concerns how that provision is governed. The purpose of this paper is to examine the governance structure of a public health board and its perceived impact on the efficacy of clinical operations.

Design/methodology/approach

Structural issues examined the level of centralisation and public participation, and whether governance should occur through elected boards or appointed managers. These issues were examined through multiple lenses. First was the intention of the structure, examining the issues identified by parliament when the new structure was created. Second, the activities of the board were examined through an analysis of board meetings. Finally, hospital clinicians were surveyed through semi-structured interviews with both quantitative and qualitative questioning.

Findings

A contradiction was revealed between intention, perception and actual activities. This raises concerns over whether the public are significantly informed to elect the best-skilled appointees to governance positions.

Practical implications

This research holds implications for selecting governance structures of public health providers.

Originality/value

Few studies have looked at the role of a publicly elected healthcare governance structure from the perspective of the clinicians. Hence, this study contributes to the literature on healthcare structure and its impact on clinical operations, by including a clinician’s perspective. However, this paper goes beyond the survey and also considers the intention of the structure as proposed by parliament, and board activities or what the board actually does. This enables a comparison of intention with outcomes and perception of those outcomes.

Abstract

Purpose – Building on the work of Wenner (2011) and Messner and Montez de Oca (2005), this study provides an updated critical stocktaking of the narrative tendencies in sport-related alcohol advertising on television. Set in contemporary understandings of a ‘crisis of masculinity’ and in the political economy of the alcohol industry, this study anchors a critical reading of masculinity, the sporting context and alcohol advertising in Wenner's (2007, 2013) ‘dirt theory of narrative ethics’.

Design/methodology/approach – Our theoretical and methodological approach is grounded in a dirt theory of narrative ethics. Set at the intersection of reader-oriented literary theory (Iser, 1978) and ethical criticism (Gregory, 1998), we ‘follow the dirt’ to understand how contagion from imported communicative meanings (McCracken, 1990) exerts power (Leach, 1976) by influencing reading and interpretation. We draw upon a diverse sample of 20 television ads representing a balanced cross-section of sport-dirtied beer commercials produced between 2010 and 2019. To balance this sample, we divided the ads by their opposing tendencies to characterize men as either ‘real men’, drawn in mythical masculinity terms, or ‘himbos’, drawn as ‘losers’ or slackers. To address the dominance of Anheuser-Busch InBev (ABI) in the American market, we further divided our ‘real men’ and ‘himbo’ samples, contrasting five ads produced for ABI brands with five ads produced for beer brands not held by ABI.

Findings – We contend that contemporary sports-dirtied beer ads combine to form a schizophrenic picture of American manhood. Male sports fans are alternatively hailed through mocking and misandry, through playfully saluting the norms of ‘bro culture’, and through encouraging men to understand themselves as proud keepers of tradition. We critically consider the ethical implications of building brand affinities through staking disparate positions in contemporary cultural and political debates about the place of men and masculinity in contemporary society.

Research limitations/implications – We discuss the difficulties involved in holding advertisers accountable for balancing ethical and market demands. Nevertheless, we call on the industry to engage in a more reflexive and responsible approach to crafting sports-dirtied alcohol advertising.

In its passage through the Grand Committee the Food Bill is being amended in a number of important particulars, and it is in the highest degree satisfactory that so much interest has been taken in the measure by members on both sides of the House as to lead to full and free discussion. Sir Charles Cameron, Mr. Kearley, Mr. Strachey, and other members have rendered excellent service by the introduction of various amendments; and Sir Charles Cameron is especially to be congratulated upon the success which has attended his efforts to induce the Committee to accept a number of alterations the wisdom of which cannot be doubted. The provision whereby local authorities will be compelled to appoint Public Analysts, and compelled to put the Acts in force in a proper manner, and the requirement that analysts shall furnish proofs of competence of a satisfactory character to the Local Government Board, will, it cannot be doubted, be productive of good results. The fact that the Local Government Board is to be given joint authority with the Board of Agriculture in insuring that the Acts are enforced is also an amendment of considerable importance, while other amendments upon what may perhaps be regarded as secondary points unquestionably trend in the right direction. It is, however, a matter for regret that the Government have not seen their way to introduce a decisive provision with regard to the use of preservatives, or to accept an effective amendment on this point. Under existing circumstances it should be plain that the right course to follow in regard to preservatives is to insist on full and adequate disclosure of their presence and of the amounts in which they are present. It is also a matter for regret that the Government have declined to give effect to the recommendation of the Food Products Committee as to the formation of an independent and representative Court of Reference. It is true that the Board of Agriculture are to make regulations in reference to standards, after consultation with experts or such inquiry as they think fit, and that such inquiries as the Board may make will be in the nature of consultations of some kind with a committee to be appointed by the Board. There is little doubt, however, that such a committee would probably be controlled by the Somerset House Department; and as we have already pointed out, however conscientious the personnel of this Department may be—and its conscientiousness cannot be doubted—it is not desirable in the public interest that any single purely analytical institution should exercise a controlling influence in the administration of the Acts. What is required is a Court of Reference which shall be so constituted as to command the confidence of the traders who are affected by the law as well as of all those who are concerned in its application. Further comment upon the proposed legislation must be reserved until the amended Bill is laid before the House.

Abstract

The brief announcement that the Government had accepted that there should be regulations on open date marking of food, to come into effect in 1975, will come as no surprise. It is a timely reminder of what public pressure can achieve these days; how sustained advocacy and publicity by interested sectors of society—magistrates, local authorities, public health workers, consumer groups—can secure legislative changes which, in this case, run counter to trade opinions and the recommendation originally made by the Food Standards Committee that such a proposal was not practical and the existing law was an adequate protection. This was stated in the FSC Report on Food Labelling of 1964, although there was no indication of the evidence reviewed or that the subject had been considered very deeply; it was, after all, only a small fraction of the problem of food labelling control. It was also stated in this Report that in certain cases, date‐stamping of food could give to purchasers a false sense of security, “not justified by the conditions under which the food has been kept since manufacture”.

Abstract

Purpose

The purpose of this paper is to report on an industry policy implementation case involving around 30 manufacturing firms, where the intellectual capital (IC) lens, and especially the intellectual capital navigator (ICN) approach, was found to be very useful for evaluating alternative servitisation strategies. Servitisation is a form of business model innovation and as such involves restructuring the firm’s resource deployment system including its IC resources.

Design/methodology/approach

The ICN was one of several methods and themes used by a sample of manufacturing firms during a 12 month period. Data capture were through video filming, observation, and formal interviewing during and after the interventions.

Findings

The ICN is considered to be the third most valuable theme in a strategic and operational servitisation programme for manufacturing firms, primarily in the domain of effectiveness evaluation of alternative resource deployment strategies and as such should be one of the key dimensions in a business model template for manufacturing firms that aim to servitize. This research also illustrates the usefulness of the intellectual capital lens in the policy implementation process.

Research limitations/implications

The findings of this study is limited to the servitization process of SME manufacturing firms in an Anglo-Saxon operating environment which very rapidly have gone from low to high cost.

Originality/value

The development of service-oriented business models for manufacturing firms suffers due to traditional business model frameworks not having a high relevance for servitising manufacturing firm. Consequently it is important to understand the potential contribution that the IC lens through the ICN can make in the servitisation process.

Abstract

Since the beginning of the 1990s, nutrition education and health promotion have increasingly focused on the influence of diet on the quality of life in old age. The Government′s Health of the Nation policy in 1991 and the COMA report on The Nutrition of Elderly People in 1992 both emphasized the need for older age groups to adopt the dietary changes recommended for the population as a whole. In order to promote healthier eating habits and consequently improve health status, it is first vital to understand what makes elderly people follow particular dietary patterns and, equally, which factors constrain their choice. Reviews the current state of research on the social, economic, psychological, physiological, educational and personal factors which mediate food choice in later life. Indications are that it is the structural influences on choice which have the greatest impact – education, income, class and access to good health care. As a result, action at national level in the form of health and social policy designed to take into account the needs of older generations is highlighted.

Abstract

Notes that the main criteria for selecting nutrients to be added to foods are that they are shown to be safe, effective and beneficial, or that for certain at‐risk population groups there is a demonstrable need. Also that addition of nutrients requires careful attention to food regulations, labelling, nutritional rationale, cost, acceptability of the product to consumers and assessment of technical and analytical limitations for compliance with label declarations. Points out that there are several examples where the nutritional enrichment of foods (restoration and fortification) has helped prevent or reduce the level of nutritional inadequacies within a population. Concludes that foods with added nutrients provide an ideal vehicle for improving the nutritional status of populations, particularly in the light of changing lifestyles and dietary patterns.
